The Ticket Central Office will use the information from this form to build your event in our ticketing software. Please note: your event must be confirmed (rental agreement fully executed and deposit received) before you are permitted to announce your event or put it on sale. It is also strongly advised that you fill out this form after meeting with the Ticket Central Office to address any questions or concerns.

IMPORTANT NOTE: The Ticket Central Office will use this information exactly how it appears to build your event. Be sure to proofread and triple-check the date and time information. Any changes requested after the event has been built and put on sale are subject to fees.
